Setting an appointment is as simple as picking up the phone and calling MMCS Clinic at 503-384-WEED (9333). We provide quality service to every adult whether you have an appointment or you are walking in for the first time.

OREGON & WASHINGTON RESIDENTS

What's the process?
For those walking in wanting an appointment you must first provide us with your medical records. You may be able be scheduled immediately provided that you have your identification and medical records in hand. You can fill out a medical records release form so we can have your records sent to our clinic.

Step 1 Get Us Your Medical Records
You have several options for getting us your medical records and every patient is entitled to their records as standard practice:

To obtain a copy of your most current medical records fill out a Medical Records Release Form (we have one or you can get one from your Physician’s office) and have the records sent to MMCS Clinic at FAX 503.281.9333. Give the Medical Records Release Form to the current medical establishment you were seen and they will fax us your records to our clinic. Need help, call us at 503-384-9333.

You can walk in to the clinic and give us your records or we can give you a Medical Records Release Form to fill out and we will fax it for you to your current medical establishment.

You can also go to the tab on the left that says Medical Records Release Form and download it and fill it out.

You can also mail your records to us at 4911 NE Sandy Blvd, Portland, OR 97213.

Please NOTE: Your medical facilities you are sending your Medical Records Release Form/s to have 30 days to get your records to us.

Privacy
We understand the importance of confidentiality, and Medical Marijuana Card Services Clinic will never disclose your personally identifying health information to anyone without YOU filling out a Medical Records Release Form authorizing that your information be released.

Step 2
Get an Appointment
Once your records are approved, one of our friendly staff will contact you to schedule your office visit with us. This call is generally completed within 24 to 48 business hours of our receipt of your medical records.

Step 3
Be Prepared for Your Appointment
Arrive 5 to 10 minutes early for your appointment and bring your identification and pay for your visit unless you have pre-paid. We accept cash, debit or credit cards. Sorry no checks, money orders or cashiers checks. Our friendly Physician/s will see you and an exit interview allows you to leave well informed with information in your hands. A typical office visit takes 30 minutes.

WHAT'S NEXT
For Oregon & Washington patients that don't have a Caregiver or Grower yet you may grow yourself. You may change Caregiver, Grower or both at any time and when changing you must notify the current Caregiver and Grower they are no longer protected under the law.

You may want to select a Caregiver and Grower in advance. For those who have a Caregiver and Grower, please have their identification photo copied front and back. 

WASHINGTON RESIDENTS
After you receive an approval from the physician you will receive your license at the end of your appointment.  Your license is good for 365 days.

OREGON RESIDENTS
Take your paperwork you just received from your appointment at MMCS Clinic to the Oregon State Building at 800 NE Oregon St, Portland, OR 97232. Enter the building and go to the cashier’s office and give them your paperwork and fee. Please feel free to contact OMMP Monday through Friday, 8am–5pm at 971.673.1234 for any help or more information.

You must provide a clear, legible copy of your current photo ID card, or a current photo and a copy of your Oregon voter’s registration card, to show that you are an Oregon resident. If the information or expiration date on the front of your ID is not current, please copy the back of the card if the back shows that it is current.

Here are directions from MMCS Clinic to the Oregon State Building:
Head southwest on NE Sandy Blvd toward NE 49th Ave. Take the ramp onto I-84. Take exit 1 toward NE Lloyd Blvd. Take a slight left at NE Lloyd Blvd, and then turn right at NE 7th Ave. Turn right at NE Oregon St. The Oregon State Building will be on the left at 800 NE Oregon Street.  It's the tall building with the dome on top.
